Please include the following information by question/topic number:
1. Why do you want me (versus another teacher) to write this recommendation?
2. Specific topic(s) of any assignment (writing, discussion, project), from the year in which you had me for English, you are particularly proud of. This may be creative or expository; however, it is good to show a balance. Also, write what you feel is important for me to note about these. Feel free to write down any mind-blowing passages from your writing or insight/point you presented. I will quote you if I agree with your assessment. I do not want the entire assignment/essay.
3. List your major contribution to the classroom environment.
4. List three or four words that you feel accurately describe you as a person and as a student.
5. Share any extenuating circumstances that prevented you from doing your best work, i.e. an extremely busy schedule either because of work or activities or course load, illness, etc.
Attach a resume or outline of your high school career (extracurricular activities, honors, awards, leadership positions, community service…)
